Tutorial: Tutorial: Evaluation Field in Forms

In this tutorial, you will see how to create a Form using the evaluation fields.

Creating the Form

Create a new application of type Form. Select the option“Single Record” and then select the table “rating_on_finish”.

Editing the Form Blocks

1. First, in the application menu, access the tab “Layout” and then select the option “Blocks”.

   

2. Use the “Create New Block” button to add a new block and then configure both according to the image below.

3. Once this is done, in the application menu, access the tab“Fields Positioning”.

4. Now, remove the “idRate” field from the display screen and move the fields in the left box, as shown below.

 

Editing Form Fields

1. In the application menu, access the tab “Fields” to find the settings for each field.

2. In the “emojis” field, we will use the data type “Rating Smile”.

3. In the “stars” field, we will use the data type “Rating” and we will enable the option to “Use Font Awesome”.

4. In the “recommend” field, we will use the data type “Rating Thumbs”.

5. Finally, select the button “Execute Application” on the toolbar to generate the Form.

After following the steps above, the Form will look like this:
